PARTICIPANT INFORMATION SHEET

The participant information sheet outlines the aims of the LEGACY study, what the study involves and frequently asked questions.

CONSENT FORM

You will be asked to sign a consent form at your first study visit to confirm that you are willing to take part in the LEGACY study. Participation is voluntary, however, and you can withdraw from the study at any time by contacting the study team.

HEALTH AND LIFESTYLE QUESTIONNAIRES

At the first study visit, you will be asked to complete a questionnaire about your personal and family medical history, medications, diet and lifestyle.

At subsequent study visits, you will be asked to complete a questionnaire that asks about changes in your health and lifestyle since your their last study visit.

At the final study visit, you will be given a study feedback questionnaire to ask about your experience participating in the LEGACY study.

FITBIT SETUP INSTRUCTIONS

You will be provided with a Fitbit at your first study visit (unless you would prefer to use one you already own). You will be asked to authorize a third party, Fitabase, to collect data from your Fitbit account, so that LEGACY research staff can access your Fitbit data.

FINGERPRICK BLOOD SAMPLE COLLECTION INSTRUCTIONS

At the start of the study you will be randomly assigned to one of two groups, which will determine the method by which you will be asked to self-collect your fingerprick blood sample during each of your study visits:

  • GROUP A: Fingerprick blood sample collection in to blood tubes.
  • GROUP B: Fingerprick blood sample collection on to ‘blood spot collection cards’.

You will be asked to self-collect your fingerprick blood sample using the same method at each visit. The fingerprick blood sample collection instructions provide information on how to collect your sample.

SALIVA SAMPLE COLLECTION INSTRUCTIONS

At each of your study visits you will be provided with a small tube to spit in to. The saliva sample collection instructions provide information on how to collect your saliva sample.

BUCCAL SWAB COLLECTION INSTRUCTIONS

At every other study visit you will be asked to self-collect a buccal swab sample. The buccal swab collection instructions provide information on how to collect your sample.

TRAVEL EXPENSES CLAIM FORM

We are happy to reimburse the following travel expenses to LEGACY study participants, up to a maximum of £15 per LEGACY study visit:

  • Public transport (e.g. bus or train)
    • Please buy standard tickets (i.e. not first-class).
  • Private transport (e.g. car or motorbike)
    • A mileage allowance of 45p per mile for car or 24p per mile for motorbike may be reimbursed.
  • Car parking
    • Car parks are available on the Cambridge Biomedical Campus and we are happy to cover the cost of the car parking ticket associated with your LEGACY study visit.

In order to refund your travel expenses, the travel expenses claim form needs to be completed and given to a member of the LEGACY research team, together with copies of your receipts.
